Health Related Professions Program Application

Certain health related degrees at PCC require prerequisites to be completed before students can apply for admission to the program.

This web page will verify that you have met the program prerequisites for your degree. If the prerequisites have been met, you will be able to request an application. All prerequisites must be completed with grades posted before a program application is mailed to you.

Only the following degrees require this form to be completed to receive a program application:

Dental Hygiene - Associate Degree
Nursing - Associate Degree
Radiologic Technology - Associate Degree
Respiratory Care - Associate Degree

If your degree is not listed above, you do not need to fill out this form.
